Welcome to JustAnswer! I am a licensed veterinarian and would be glad to help!
What you are likely describing is an Acral Lick GranulomaLINKIn cases like this, it is often allergy related, although pain and anxiety are also known to play a role. The treatment is often a combination of antihistamines and steroids, antibiotics for secondary infections as well as testing such as x-rays to make sure there is not a local bone problem or other soucre of pain leading to the lickingif OCD, then medications such as Prozac are used.
Prozac was not even mentioned by my vet. She is a wonderful dog. Never alone and is loved richly. She sleeps with me and my husband (curled into my legs) every night. I am worried this will never end.
The above link will help as this is what your description is consistent with.
